ACL National Chronic Disease Self-Management Education Resource Center

Funding Agency:
Department of Health and Human Services

Through this funding opportunity, the Administration on Aging (AoA), part of the Administration for Community Living (ACL), plans to award one cooperative agreement to support a National Chronic Disease Self-Management Education (CDSME) Resource Center (the “Center”). This funding, from the 2021 Prevention and Public Health Fund, will support $2,000,000 each year of a five-year project period, contingent upon the continued availability of federal funds.

The Center will provide technical assistance, education, and resources for the national CDSME network of partners to increase the number of older adults and adults with disabilities who participate in evidence-based CDSME and self-management support programs, and support the integration and sustainability of these programs within community integrated health networks.
