The purpose of the Healthy Tomorrows Partnership for Children (HTPCP) Program is to promote access to health care for under-resourced and underserved children, youth, and their families nationwide by supporting the implementation and evaluation of innovative community-based programs and models of care that strengthen collaborative work between local pediatric providers and community partners to provide quality health care and preventative services; and promote the long-term sustainability of community-based programs.

Eligibility Requirements

Any domestic public or private entity, including an Indian tribe or tribal organization (as defined at 25 U.S.C. § 450b), is eligible to apply for federal funding under this notice. See 42 CFR § 51a.3(a). Domestic community-based organizations, including faithbased organizations, are eligible to apply.

NOTE: Multiple applications from an organization with the same DUNS number or Unique Entity Identifier (UEI) are allowable if the applications propose separate and distinct projects.
